Our school Medication ADMINISTRATION Form must accompany the medication and be completed and signed by your child’s
physician, and signed by the parents.The form also applies to over -the -counter medications including Tylenol, Advil, and cough drops.
No medication will be accepted if not in it's original container. Medications WILL NOT be accepted in plastic baggies.ATTENTION PARENTS
All medication MUST BE DROPPED OFF TO THE NURSE IN THE HEALTH OFFICE ON THE FIRST DAY OF SCHOOL.THIS INCLUDES ALL OVER THE COUNTER AND PRESCRIPTION MEDICATION, EPI-PENS AND INHALERS**ATTENTION 8TH GRADE PARENTS**
Attention incoming 6th grade parents
- All medication for the 8th grade trip MUST be brought into the health office by DATE to be determined 2024.
- Medication forms are REQUIRED to have medication administered to your child.
- All medications administered in school/ OR on trips must be brought in by the parent/guardian.
