**ATTENTION 8TH GRADE PARENTS**
Our school Medication ADMINISTRATION Form must accompany the medication and be filled out and signed by your child’s physician, and signed by the parent.The form also applies to over -the -counter medications including Tylenol, Advil, and cough drops.
- All medication for the Philadelphia trip MUST be brougt into the health office by Friday May 31, 2019.
- Medication forms are REQUIRED to have medication administered to your child.
- All medications administered in school/OR on trips must be brought in by the parent/guardian
- In the original container and be clearly labeled.
SPORTS UPDATE**Any student who plans on participating in a SPRING sport here at VMS must have an updated Pre-participationPhysical Evaluation form AND History Form on file in the health office.Please see the nurse to make sure all the proper forms are completed.
