     **ATTENTION 8TH GRADE PARENTS**

     
    • All medication for the Philadelphia trip MUST be brougt into the health office by Friday May 31, 2019.
    • Medication forms are REQUIRED to have medication administered to your child.
    • All medications administered in school/OR on trips must be brought in by the parent/guardian
    • In the original container and be clearly labeled.  
     
    Our school Medication ADMINISTRATION Form must accompany the medication and be filled out and signed by your child’s physician, and signed by the parent. 
    The form also applies to over -the -counter medications including Tylenol, Advil, and cough drops.

     SPORTS UPDATE

    **Any student who plans on participating in a  SPRING sport here at VMS must have an updated Pre-participation
    Physical Evaluation form AND History Form on file in the health office.   
    Please see the nurse to make sure all the proper forms are completed. Forms can be found on my website. 
     
    **All Spring Sport Pre-participation and History forms are due no later  then March 15, 2019
     

     

    No medication will be excepted if not in it's original containers. Medication will not be accepted in plastic baggies
     
     
    Attention incoming 6th grade parents

    The New Jersey Department of Health requires all student's entering the sixth grade to receive
    2 immunizations:

    Tdap
    Meningoccal
    ALL Immunization RECORDS and Sports Athletic Forms are due to the health office
    by August 15, 2018. Any questions or concerns PLEASE contact me at 201-337-8185. 
     
     
     ATTENTION PARENTS

    All medication MUST BE  DROPPED OFF TO THE NURSE IN THE HEALTH OFFICE ON THE FIRST DAY OF SCHOOL.
    THIS INCLUDES ALL OVER THE COUNTER AND PRESCRIPTION MEDICATION, EPI-PENS AND INHALERS 


     
    Physical Examinations for High School
     
    ALL current eighth grade students must have a current physical to enter high school. The
    examinations must be within the last 
    365 days of 
    the start of the school year.
     



    Attention  6th grade parents
    ALL physical and immunizations forms are now overdue! 
     
    AMMENDED IMMUNIZATION REGULATIONS
    Children who attend a licensed child care center and preschoolers MUST receive annual influenza vaccinations AND a pneumoccocal vaccine.
    In addition, children born after January 1, 1997 and enrolled in Grade 6 or transferring into a New Jersey school from another state or country
    will be required to receive a booster dose of the diptheria, tetanus and pertussis vaccine as well as one dose of a 
    meningoccocal vaccine
