EMERGECNY CARDS
ATTENTION PARENTS-
It is required to have an Emergency card on file for each student. Please make sure you complete the Student Emergency Health and Accident form that was sent home with your child and have your child return it to the health office.
AMMENDED IMMUNIZATION REGULATIONS
Children who attend a licensed child care center and preschoolers MUST receive annual influenza vaccinations AND a pneumoccocal vaccine.In addition, children born after January 1, 1997 and enrolled in Grade 6 or transferring into a New Jersey school from another state or country.will be required to receive a booster dose of the diptheria, tetanus and pertussis vaccine as well as one dose of a
meningoccocal vaccineAdministration of Medication in school and on school-sponsored tripsOur school Medication ADMINISTRATION Form must accompany the medication and be completed and signed by your child’s
physician, and signed by the parent.
The form also applies to over -the -counter medications including Tylenol, Advil, and cough drops.No medication will be accepted if not in it's original containers. Medications will not be accepted in plastic baggies.
ATTENTION PARENTSAll medication MUST BE DROPPED OFF TO THE NURSE IN THE HEALTH OFFICE ON THE FIRST DAY OF SCHOOL.
THIS INCLUDES ALL OVER THE COUNTER AND PRESCRIPTION MEDICATION, EPI-PENS AND INHALERS**ATTENTION 8TH GRADE PARENTS**- All medication for the Philadelphia trip MUST be brought into the health office by Friday ______, 2021.
- Medication forms are REQUIRED to have medication administered to your child.
- All medications administered in school/OR on trips must be brought in by the parent/guardian.
- In the original container and be clearly labeled.
