Registration for New Students to Oakland Public Schools
Please contact the Oakland Public Schools Registrar to schedule an appointment.

You will be required to bring the following documents to your appointment:
1. Completed Enrollment/Application Form
2. If the resident owns the home, a recorded deed is required or a copy of a current tax bill/statement.
3. If renting, an original lease, dated and signed, effective during the current school year. If no original lease is available, affidavit forms are available on the website. Please refer to the Affidavit form for more information
4. A copy of a current utility bill (i.e., PSE&G, Phone, Water, Cable) with the name of the Oakland residency shown. If you have just moved in, please provide a work order for one of the above.
5. One additional proof of residency with the name of the Oakland residency shown. Such as voter registration, driver's license, financial account statement, utility bills, court orders, documents pertaining to military status.
6. If the child that is being registered is part of a shared custody agreement, please provide evidence of the Oakland residence being the primary parent or the court order/agreement used to determine residency.
7. Birth Certificate (original) of the child, if born in the U.S. Passport only if born outside the U.S. Children shall be admitted to kindergarten provided he/she will have attained the age of five years on or before October 1 of the year in which admission is requested.
8. Physical Examination Form and Immunization Form with up-to-date immunizations must be provided and reviewed by the school nurse before the student may enter school.
*Additional forms will be required for life threatening allergies, asthma, diabetes, and seizure disorder. Please contact the school nurse for further information.
